You can use a simple QR code generator extension. If it’s not installed yet:
- Install the Barcode Generator extension that generates a QR code.
- Click the icon of the installed extension and select the 'QR code' format.
- Press ESC to close the extension window for now.

First, you need to get the link to the form:
- Go to your desired Google Form or create a new one there.
- Click the "Send" button (in the top right corner), it will show the sharing options.

Select the link icon (chain), and the desired link will appear below the 'Link'
label.
- Click the link text to highlight the line.
- Or: You can also check 'Shorten URL' checkbox, this will shorten and also highlight the link line, which makes the future QR code more compact.

Create the QR code:
- Click the Barcode Generator extension icon.
- The required QR code will be generated immediately and copied to the clipboard.

- Place the QR code in a presentation, on a poster, or on a website.
- Now when users encounter your Google Forms QR code, they can scan it and proceed to the form.
